Provide counseling, discharge planning and treatment assistance to patients in the Tellurian Detox facility.
PATH (Projects for Assistance in Transition from Homelessness) is a street outreach program serving single adults in Madison and Dane County who are experiencing homelessness or are at risk of homelessness and have significant mental health barriers. The PATH Outreach Specialist provides outreach, assessment, information and referral, short-term case management, service planning, housing support/advocacy, and SSI/SSDI application assistance to program participants. The end goal is permanent, stable housing with community support to remain in housing.
Under supervision, the On-Call Human Services Specialist is responsible for assisting with the daily functions of the program units and patient care working within a treatment team model. The On-Call Human Services Specialist, following AODA/Mental Health program policies and procedures, provides direct services to clients and follows through on previously determined AODA/Mental Health goals. The Human Services Specialists must conduct themselves ethically and professionally in keeping with program and company philosophies and policies. High-quality professional relationships as exemplified by honesty, respect, and fairness must be always maintained.

The Case Manager works in a multi-disciplinary team providing community case management and service coordination for adults with mental illness and/or substance use disorders. This is a fast paced, direct service position with the majority of services being provided in the community. Areas of focus may include symptom management, medication monitoring, housing, medical care, psychiatric care, crisis management, and benefits coordination.
